Meditative traditions were transmitted, almost without exception, inside religious institutions, monasteries, ashrams, lineages of ordained teachers, where a student's practice was shaped by ongoing supervision and an interpretive framework the tradition itself supplied. That began to change in the colonial and postcolonial periods, as reform-minded teachers in Asia, responding partly to Western interest and partly to internal reform movements, began presenting meditation to lay audiences and eventually to Western students outside monastic structures. The most consequential modern development came from clinical medicine rather than religion: Jon Kabat-Zinn, trained in part by Zen and vipassana teachers, founded Mindfulness-Based Stress Reduction at the University of Massachusetts Medical School in 1979, adapting Buddhist attention practice into a structured, largely secular eight-week program aimed at chronic pain and stress. That program and its many descendants have made a technique derived from Buddhist meditation into one of the most widely taught practices in medicine, education and corporate wellness, generally without the ethical framework, cosmology or teacher relationship the Buddhist tradition builds around it.

Scholars such as Robert Sharf have argued this represents a real transformation of the practice's meaning, not merely a simplification, since concepts like mindfulness were reworked to fit a modern therapeutic vocabulary that the source traditions did not use in the same way. Traditional Buddhist teachers are divided on whether this secular offshoot serves as a useful entry point or represents a loss of the practice's actual purpose.
