Tatbir, striking the head or back with a blade or chain until blood is drawn, is the most visually extreme element of Ashura mourning and also the most contested within Shia Islam itself, not only by outsiders. Senior clerical authorities, including Iran's Supreme Leader Ali Khamenei and Lebanon's Hezbollah leadership, have issued rulings discouraging or banning the practice, arguing that it damages the image of Shia Islam, that it has no basis in the earliest mourning traditions, and that it causes needless harm the faith does not require, positions that put major religious and political authorities on the same side as external critics who call it self-harm.

The practice nonetheless persists in parts of Iraq, Lebanon, Pakistan and India among communities that regard it as an authentic expression of grief and defend it against clerical discouragement, so the argument has not been settled by the rulings against it. A widely promoted alternative has grown alongside the dispute: donating blood during the mourning period, organised by Shia communities and blood banks in several countries as a substitute act that channels the same impulse toward self-sacrifice into a measurable public benefit, and this substitution is now actively encouraged by clerics who oppose tatbir as the preferred way to mark the day. Chest-beating in unison, by contrast, draws no comparable controversy and remains the near-universal form of physical mourning across Shia communities regardless of their position on the sharper practice.
