By 2011, Ireland had already read the Ferns Report and the Ryan and Murphy Reports, each documenting how Catholic dioceses and residential institutions had failed children in their care. The Cloyne Report, into the Diocese of Cloyne in County Cork, was expected to close that chapter. Instead it opened a sharper one.

The Commission of Investigation, chaired by Judge Yvonne Murphy, the same judge who had led the Dublin inquiry two years earlier, examined how the diocese handled complaints against 19 priests between 1996 and 2009, well after Ireland's bishops had adopted their own child protection framework in response to the earlier reports. What it found was not ignorance of the rules but a diocese that had told the government it was following them while it was not: Bishop John Magee had assured the state that every allegation was being passed to the civil authorities, and the commission found this was false. Magee resigned as Bishop of Cloyne in March 2010, offering a public apology that asked forgiveness of anyone he had failed or caused to suffer and accepting responsibility for the diocese's handling of the allegations during his tenure.

The report also uncovered a 1997 letter from the Vatican's representative in Ireland describing the Irish bishops' own reporting framework as a set of guidelines rather than binding policy, a characterization that gave individual bishops room to treat compliance as optional. The finding was significant enough that it changed diplomatic relations: within months, the Irish government closed its embassy to the Holy See, a rupture that had not occurred over any of the three earlier reports. Where Ferns, Ryan and Murphy had documented failure inside a national church, Cloyne documented a failure that reached Rome itself.
